  • Update your personalized prevention plan.
  • Screen for depression and cognitive impairment.
  • Review functional safety and advance care planning.

Hemendra Sarda MD, MPH

Dr. Hemendra Sarda is a board certified family medicine physician. He graduated from medical school from Smt. NHL Municipal Medical College, Ahmedabad, India. He pursued Masters in Public Health (MPH) from Missouri State University, Springfield, Missouri. He completed his residency in family medicine from Carilion Clinic, Roanoke, Virginia. He continued working at Carilion Clinic until 2020 and then moved to Florida. During his career, he held several leadership positions including associate medical director, medical director and chair of various hospital committees.

Suchet Sarda MD, FACP, MMM

Dr. Suchet Sarda is a board-certified Internal Medicine physician dedicated to providing compassionate, patient-centered care. With more than a decade of experience, she believes the best care starts with truly listening and understanding each patient’s unique health needs.She completed her residency at Carilion Clinic in Roanoke, Virginia, and earned a Master of Medical Management from Carnegie Mellon University.Patients appreciate her friendly approach, clear communication, and focus on building long-term relationships.

Absolutely. We encourage family members or caregivers to be present, especially if the patient has hearing loss, memory concerns, or a complex medication schedule. With the patient’s consent, having an extra set of ears helps ensure that care instructions are understood and followed.

To discuss your loved one’s health over the phone or in person, we must have a signed HIPAA Authorization Form on file. Once that is established, we can share lab results, medication changes, and treatment plans with you directly.

Sudden confusion (delirium) or mood changes can often be caused by treatable medical issues, such as a Urinary Tract Infection (UTI) or a medication interaction. Please call the office immediately if you notice a sharp decline so we can rule out acute infections.

Over-medication is a significant risk for seniors. We perform regular “medication reconciliations” where we review every bottle. My goal is to deprescribe unnecessary medications to reduce side effects, lower costs, and prevent dangerous interactions.

Yes. These conversations are a standard part of our Medicare Annual Wellness Visits. We can help your family navigate the medical aspects of a Power of Attorney or Living Will to ensure your loved one’s healthcare wishes are honored.
