Stay ahead of your health with personalized annual wellness exams. As your primary care physician, we offer dedicated one-on-one time to move beyond “sick care” and focus on a long-term prevention strategy.
Medicare Annual Wellness Visits (AWV)
If you have been enrolled in Medicare Part B for at least 12 months, you are eligible for a yearly Wellness Visit at no out-of-pocket cost . Unlike a traditional physical, this visit is a planning session to create or update a personalized prevention plan. We will:
Perform a health risk assessment to identify potential concerns early.
Conduct a cognitive assessment to check for signs of memory loss or dementia.
Establish a 5-10 year screening schedule for services like mammograms or colonoscopies.
Review functional ability and safety, including screenings for hearing and fall risks.
Patient Prep Checklist
To make the most of our time, please bring the following to your appointment:
Health Documentation: Your Medicare card, photo ID, and any current immunization records.
Medication List: A full list of all prescriptions, over-the-counter drugs, vitamins, and supplements (including dosages).
Care Team Contact Info: Names and phone numbers of any specialists or other healthcare providers you see.
Family History: A detailed history of health issues among your parents, siblings, and children.
Advance Directives: Copies of any legal documents, such as a living will or power of attorney, for review.

Hemendra Sarda MD, MPH
Dr. Hemendra Sarda is a board certified family medicine physician. He graduated from medical school from Smt. NHL Municipal Medical College, Ahmedabad, India. He pursued Masters in Public Health (MPH) from Missouri State University, Springfield, Missouri. He completed his residency in family medicine from Carilion Clinic, Roanoke, Virginia. He continued working at Carilion Clinic until 2020 and then moved to Florida. During his career, he held several leadership positions including associate medical director, medical director and chair of various hospital committees.
Suchet Sarda MD, FACP, MMM
Dr. Suchet Sarda is a board-certified Internal Medicine physician dedicated to providing compassionate, patient-centered care. With more than a decade of experience, she believes the best care starts with truly listening and understanding each patient’s unique health needs. She completed her residency at Carilion Clinic in Roanoke, Virginia, and earned a Master of Medical Management from Carnegie Mellon University. Patients appreciate her friendly approach, clear, authentic, accurate, comprehensive communication, and focus on building long-term relationships.

What is an Annual Wellness Exam?
An Annual Wellness Exam is a preventive visit focused on developing or updating a Personalized Prevention Plan. Unlike a sick visit, its goal is to prevent disease and disability based on your current health and risk factors.
What is the difference between a Wellness Visit and a Physical Exam?
While they sound similar, they serve different purposes. A Wellness Visit is a “talk-based” appointment focusing on health risk assessments, screenings, and long-term planning. An Annual Physical is more “hands-on,” where a doctor physically examines you to find or treat existing medical issues.
Is the exam covered by insurance?
Most insurance plans, including Medicare Part B, cover one wellness visit every 12 months at no out-of-pocket cost to you. However, if you discuss new or chronic medical problems during the visit, your insurer may apply a co-pay or deductible for those specific services.
What should I bring to my appointment?
To make the most of your visit, please bring the following:
A full list of your current medications and supplements, including dosages.
Your immunization records.
A detailed family health history.
A list of other healthcare providers or specialists you see.
What happens during the visit?
Your provider will typically:
Record routine measurements like height, weight, and blood pressure.
Perform a cognitive assessment to look for signs of memory loss or dementia.
Screen for depression and evaluate your risk of falls.
Review your Health Risk Assessment questionnaire.
Provide personalized health advice and a schedule for future screenings.
Do I need to fast before my exam?
Fasting is usually not required for a standard Wellness Visit because it does not typically include blood work. However, if you are also scheduled for a physical or specific lab tests, we may ask you to fast. Check your appointment confirmation or contact our office to be sure.
